School-Based Health Centers, based in six schools, are a partnership of the Portland Public Health Division, Portland Public Schools and Maine Medical Center working together to keep Portland students healthy.

All services are confidential. We also offer translation services for ESL students.

SBHCs provide students on-site health care services including:

Dental health screenings and preventive treatments also are available in selected schools. For more information on these services, please click here.

Please note: The School-Based Health Centers do not take the place of your child’s doctor and other health care providers. We work with primary care physicians to better coordinate your child’s health care. Ongoing communication occurs as necessary between the School-Based Health Center, and your child’s doctor.

FAQs

What kind of health care providers are on staff at the Centers?
The School-Based Health Center staff consists of pediatricians, nurse practitioners, clinic assistants, dental hygienists, dental assistants, mental health counselors and substance abuse counselors. School nurses and social workers work closely with the Center staff to coordinate appointments and referrals.

What students are eligible?
We are happy to see any student with a completed enrollment form that has been signed by a parent. Students who are at least 18 years old, emancipated minors, married, or who live independently may sign permission for themselves.

How do I enroll my child?
Please complete an enrollment form for your child and deliver it to their school nurse. If you have access to a printer, please download the form here, or you can pick one up in the school nurse’s office:

Grades K-5
Grades 6-12

How much do the services cost?
We bill Medicaid, Maine Care and private insurance if applicable to cover costs of services. No one will be denied care if unable to pay.

What are the locations and hours of the School-Based Health Centers?
The School-Based Health Centers are open several days a week and have appointments available both during and after school. Walk-in services are provided as available. See the school nurse to set up a time that is right for you.
