Senior Outreach Nursing Program
Senior Outreach Nursing Program:

Nursing visits are provided free of charge to low-income Portland residents over the age of 50, who are not eligible for home health services under any other funding program. The City's Outreach Nurse is available to schedule blood pressure clinics and to visit eligible clients in their own homes.

Services include:

In addition to providing professional nursing oversight and interventions, the Outreach Nurse offers reassurance and support for vulnerable seniors who live alone and may be otherwise isolated.
